I heard that resorts and liveaboards offer this as something to do, but what does it consist of exactly?

Island hopping basically takes you around other islands, weather it is another resort, an uninhabited island or an inhabited island, all in a day-tour designed to give you a taste of the country. When you visit these places you get the chance to do some snorkelling there, have a barbecue on the beach and many other things. Some resorts even do the island hopping by sea plane.
